In the West Liberty Student Health Portal you will fill out the required medical documentation for participation in athletics and the necessary documentation for attendance at the university. The information you provide is secure and confidential and will be accessible by the Athletic Training Staff and Student Health Services.
*If you are a minor, please contact your athletic trainer to get documentation for your Guardian to sign. WLU Athletic Trainers, the sports they cover, and their contact information can be found HERE.
MEDICAL FORMS
The following forms are available here for your convenience. They are also available in the West Liberty Student Health Portal along with an explanation when each needs to be used. The physical is a required for ALL athletes, New and Returning. Each of the other forms here may or may not be necessary for you. To determine if you need to print these forms out for your Physician to fill out, visit the West Liberty Student Health Portal and follow the directions.
